Practice Area

We accept patients living in the SS6 Postcode area – Rayleigh. That is the area bounded by Carpenters Arms roundabout in the West, Miller and Carter to the East, Rayleigh Weir roundabout to the South and the Hockley boundary to the North. You are welcome to register with us and our reception staff will be happy to guide you through the procedure.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register. Patient needs to come into the surgery with 2 forms of ID and book a new patient health check once they are registered.

Paper forms are still available if you need one.

All new adult patients are required to have a New Patient Health Check these are completed by the Practice Nurse or Pharmacist.

Please help us

If your name has changed, you will need to provide the surgery with a copy of Change of Name Deed.

Please help us to keep our records up to date by telling the receptionist promptly if you change your name, address or telephone number. If you have been away from the area for a while, or if there are other questions about your registration, you may be asked to produce your NHS Medical card. Please find it and keep it in a safe place!

Medical treatment is available from the date of registration. Please contact reception for further information.

The National Care Record Service (NCRS)

Important information regarding your NHS medical records – Summary Care Record

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Zero Tolerance

We DO NOT tolerate violent or abusive language or actions towards staff or premises. Any such person will be asked to leave the premises (with the assistance of the Police if necessary) and may also be removed from the Doctors’ list.
